To begin the enrollment process for your child(ren) kindly download and complete the following forms. Please note: Filling out these forms does not mean that your child is enrolled.

ENROLLMENT CHECKLIST:

1. Registration/ Emergency Contact Form: Each child must have a completed registration form signed by a parent or guardian.
2. Authorization for Emergency Care: Emergency form must be signed before the child enrolls.
3. Child Information/ History Form: Completed and signed.
4. Health Forms: The State of New Hampshire requires immunization records and health forms signed by the child’s physician and parents.
5. Casa dei Bambini Tour: Please contact us to schedule an appointment for a tour of our center and review our program. Enrollment of your child is subject to space availability. Please contact the center early to ensure placement.
6. Contractual Agreement Form: Completed and signed.
