To begin the enrollment process for your child(ren) kindly download and complete the following forms or contact the school to request an enrollment packet. Please note: Filling out these forms does not mean that your child is enrolled.

ENROLLMENT CHECKLIST: 

1. Registration/ Emergency Contact Form: Each child must have a completed registration form signed by a parent or guardian. 
2. Authorization for Emergency Care: Emergency form must be signed before the child enrolls. 
3. Child Information/ History Form: Completed and signed. 
4. Health Forms: The State of New Hampshire requires immunization records and health forms signed by the child’s physician and parents. These records must be on file by September 10^th at the Meeting House Montessori School. 
5. Meeting House Montessori School Tour:  Please contact us to schedule an appointment to tour our school and review our program. Enrollment of your child is subject to space availability. Please contact the school early to ensure placement.
6. Contractual Agreement Form: Completed and signed.
